Why this applies to Finasteride

Finasteride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the short version: your body uses a liver enzyme called CYP3A4 to help break down finasteride. In theory, if milk thistle slowed that enzyme down, a bit more finasteride could linger in your system. With finasteride that might mean a little more of its known effects, like reduced sex drive, trouble with erections, or breast tenderness.

That said, I wouldn't lose sleep over this one. The evidence is weak and conflicting, mostly lab and animal work, and human studies haven't shown milk thistle meaningfully changing CYP3A4 drugs. Finasteride isn't a prodrug, so it doesn't get flipped, and it isn't a narrow-margin medicine. Still, it's always smart to tell your pharmacist or doctor what supplements you take so they can keep an eye on things.

Milk thistle is a popular herbal supplement most often used for liver health, and its main active component is a group of compounds called silymarin. While it is generally well tolerated, the scientific evidence that it meaningfully improves liver disease or other conditions is mixed and far from conclusive. About Finasteride

What Is Finasteride Used For?

Brand names: Propecia, Proscar

Finasteride tablets (including the brand Proscar) are used in men with an enlarged prostate (benign prostatic hyperplasia, or BPH) to improve urinary symptoms, lower the risk of acute urinary retention (a sudden inability to urinate), and reduce the need for prostate surgery. Finasteride tablets may also be used alongside the alpha-blocker doxazosin to slow the progression of BPH symptoms.

Propecia (and finasteride 1 mg coated tablets) is indicated for the treatment of male pattern hair loss (androgenetic alopecia) in men only. Propecia is not approved for use in women, and its effectiveness for hair loss at the temples has not been established. Finasteride is not approved for the prevention of prostate cancer.
