Insulin falls under ANTIDIABETES DRUGS — the group Milk Thistle interacts with.

Taking milk thistle with antidiabetes drugs may increase the risk of hypoglycemia.
Clinical research shows that milk thistle extract, alone or along with tree turmeric extract, can lower blood glucose levels and glycated hemoglobin (HbA1c) in patients with type 2 diabetes, including those already taking antidiabetes drugs. Additionally, animal research shows that milk thistle extract increases the metformin maximum plasma concentration and area under the curve and decreases the renal clearance of metformin, due to inhibition of the multi-drug and toxin extrusion protein 1 (MATE1) renal tubular transport protein.

Here's the simple version: milk thistle may gently lower your blood sugar on its own. Some studies in people with type 2 diabetes found that silymarin (milk thistle's main active part) brought blood sugar and HbA1c down a bit. Insulin already lowers your sugar, so stacking the two could push your levels too low.

For insulin like NovoRapid, that means a higher chance of hypoglycemia (low blood sugar): shakiness, sweating, a fast heartbeat, dizziness, confusion, or feeling suddenly hungry. The research here is moderate quality, not rock-solid, so this won't happen to everyone. If you'd like to try milk thistle, check your sugars a little more closely at first and talk with your doctor or pharmacist before changing anything, since your insulin dose may need a look.
