Norgestrel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the short version: your body uses an enzyme called CYP3A4 to help clear norgestrel, the progestin in your mini-pill. In theory, if milk thistle slowed that enzyme down, a little more of the hormone could hang around, which might mean more spotting, breast tenderness, or mood changes. But honestly, the evidence here is thin.

Most of the good human studies show milk thistle does not meaningfully change CYP3A4, and norgestrel is the active hormone (not a prodrug), so there's no on/off switch to flip. This is a minor, unlikely interaction based mostly on lab and a few cancer-drug case reports, none of which involved birth control. I wouldn't expect your pill to fail or become unsafe. Still, since reliable contraception matters, mention any milk thistle to your pharmacist or doctor.

Milk thistle is a popular herbal supplement most often used for liver health, and its main active component is a group of compounds called silymarin. While it is generally well tolerated, the scientific evidence that it meaningfully improves liver disease or other conditions is mixed and far from conclusive. What Is Progestin-Only (norethindrone) Oral Contraceptives Used For?

Brand names: Ovrette, Orval 21

Norethindrone progestin-only pills — sometimes called the "mini-pill" — are used solely to prevent pregnancy. They do not contain estrogen, which makes them an option for women who cannot or prefer not to use estrogen-containing contraceptives.

When taken perfectly (same time every day, no missed pills), fewer than 1 in 100 women will become pregnant in the first year. With typical use, that number rises to about 9 in 100 — mostly because of late or skipped pills. Consistency is everything with this method.
