Why this applies to Fluticasone furoate

Fluticasone furoate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist for fluticasone furoate (Avamys nasal spray or Arnuity Ellipta inhaler) and milk thistle. Your body clears this steroid using a liver enzyme called CYP3A4. The theory is that milk thistle might slow that enzyme down a little, which could let slightly more fluticasone hang around. In plain terms, that might nudge up steroid-type effects.

Honestly though, this is a weak, mostly lab-based concern, and the human studies we have don't show milk thistle meaningfully blocking CYP3A4. Fluticasone furoate also acts mostly right at the nose or lungs, so very little reaches the rest of your body. For most people this combo is unlikely to cause trouble. Still, it's worth a quick word with your pharmacist or doctor before adding the supplement.
