Why this applies to Aprepitant

Aprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist: your body uses a liver enzyme called CYP3A4 to break down aprepitant. In lab dishes, plumbagin (the active compound in Ceylon Leadwort) appears to slow that enzyme down. If that happened in people, aprepitant could clear more slowly and build up a bit, which might mean more of its usual side effects like tiredness, hiccups, headache, or loose stools.

I want to be honest about the strength of this, though. This warning comes only from test-tube research, and nothing has actually been reported in real patients. So it's more of a theoretical caution than a proven problem. Since aprepitant is part of a chemo support plan, please run any herbal supplement past your oncology team or pharmacist before mixing the two.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Aprepitant

What Is Aprepitant Used For?

Brand names: Emend, Cinvanti

Aprepitant is used to prevent nausea and vomiting — not to treat it once it has started. It is always used alongside other anti-nausea medicines, not on its own.

EMEND capsules (oral) are approved for adults and children 12 and older to prevent nausea from highly or moderately emetogenic (nausea-causing) chemotherapy. EMEND for oral suspension extends that same use down to children as young as 6 months. CINVANTI (IV injection) is approved for adults only, for chemotherapy-related nausea. APONVIE (IV emulsion) is approved for adults to prevent postoperative nausea and vomiting (PONV) — nausea after surgery. Aprepitant capsules (generic) are approved for adults and children 12 and older for chemotherapy-related nausea, and also for adults to prevent PONV.
