Why this applies to Busulfan

Busulfan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist for Busulfan: your body partly relies on a liver enzyme called CYP3A4 to clear this chemotherapy drug. A compound in Ceylon Leadwort, plumbagin, has been shown to slow that enzyme down. In theory, that could let busulfan build up to higher levels than expected, which matters because this is a powerful, narrow-margin drug where small changes can swing toward more toxicity (think low blood counts, mouth sores, nausea, liver strain, or lung problems).

I want to be honest about the strength here: this is only seen in lab studies, not in real patients, so it's theoretical. Still, busulfan isn't a drug to take chances with. Please don't start or stop anything on your own. Loop in your oncology team or pharmacist before adding this supplement.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Busulfan Used For?

Busulfan is used to treat chronic myelogenous leukemia (CML), a cancer of the blood and bone marrow. The specific use depends on the form and product.

Myleran (oral tablet) is used for the palliative treatment of CML — meaning it helps control the disease and relieve symptoms. The intravenous injection forms (Busulfex and Busulfan Injection) are used in combination with another chemotherapy drug, cyclophosphamide, as a conditioning regimen to prepare the body before an allogeneic hematopoietic progenitor cell transplant (a bone marrow or stem cell transplant from a donor) for CML.
