Here's the wrinkle with this combo: the codeine part of your pill is a prodrug. Your body has to use an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. A compound in Ceylon Leadwort, plumbagin, has been shown to slow that same enzyme down in lab tests. If it did that in your body, you might convert less codeine into morphine, so you could get weaker pain relief from this medication.
The good news is this is only a theoretical concern based on test-tube studies, not real-world reports in people, so I wouldn't lose sleep over it. Still, before adding Ceylon Leadwort, it's worth a quick chat with your pharmacist or doctor.

In this combo pill, the part that matters most for this interaction is the caffeine. Your body relies on an enzyme called CYP1A2 to clear caffeine out of your system. Lab studies suggest plumbagin, a compound in Ceylon Leadwort, may slow that same enzyme down. In theory, that could let caffeine linger longer, so you might feel more jittery, have a racing heart, trouble sleeping, or an upset stomach.
I'd put this in the possible but not proven bucket. It's only been seen in test tubes, never reported in real people, so I don't want to alarm you. The codeine and acetaminophen aren't really affected here. If you're considering this supplement, just run it by your pharmacist or doctor first.

Here's the thing about this combo pill: the codeine part is actually a prodrug. Your body has to convert it into morphine to get pain relief, and that conversion mostly runs through a different enzyme (CYP2D6), not CYP3A4. The lab data we have suggests plumbagin in Ceylon Leadwort can slow down CYP3A4, which is one of the side paths codeine uses. In theory, that could shift a little more codeine toward becoming morphine, which might mean stronger effects like drowsiness, constipation, or slowed breathing.
I want to be honest though: this is only test-tube research, never confirmed in real people, so I wouldn't lose sleep over it. The acetaminophen and caffeine pieces aren't a big concern here. Still, please run it by your pharmacist or doctor before pairing the two.

Here's the piece that matters in this combo product: the acetaminophen part is partly handled by a liver enzyme called CYP2E1. In lab dishes, plumbagin (a compound in Ceylon Leadwort) appears to slow that enzyme down. In plain terms, that could let a small amount of acetaminophen shift down a pathway that produces a liver-stressing byproduct, so the theoretical concern is more strain on the liver, especially at higher acetaminophen doses or with alcohol.
I want to be honest about the strength here: this is only test-tube evidence and has not been seen in real people, so I wouldn't lose sleep over it. Still, because acetaminophen and the liver are involved, it's worth a quick chat with your pharmacist or doctor before pairing them, and never exceed your acetaminophen limit.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts.
