Mexiletine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist for mexiletine. Your liver uses an enzyme called CYP2D6 to help break this medication down and clear it from your body. A compound in Ceylon Leadwort called plumbagin appears to slow that same enzyme. If CYP2D6 is dialed back, mexiletine could build up to higher levels than expected, and since it's a heart rhythm drug with a narrow safety window, even modest changes deserve attention. Higher levels could mean more side effects like dizziness, nausea, tremor, or in rare cases new rhythm problems.

That said, this is based only on lab (test tube) research, not real people, so I don't want to overstate it. Still, before combining the two, please run it by your pharmacist or cardiologist first.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the basic idea with mexiletine. Your liver uses an enzyme called CYP1A2 to clear this drug out of your body. A compound in Ceylon Leadwort called plumbagin has been shown to slow that same enzyme down, at least in lab dish testing. If it slowed it down in a real person, mexiletine could build up higher than expected. Since mexiletine isn't a prodrug (it's already active), more drug hanging around means a stronger effect and a bigger chance of side effects like nausea, dizziness, tremor, blurred vision, or a worsening of your heart rhythm.

Now the honest part: this is only theoretical and has not been seen in people. Still, because mexiletine has a narrow safety window, please loop in your pharmacist or doctor before combining them.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Mexiletine

What Is Mexiletine Used For?

Brand name: Mexitil

Mexiletine Hydrochloride capsules are indicated for the treatment of documented, life-threatening ventricular arrhythmias — for example, sustained ventricular tachycardia (a dangerously fast heart rhythm originating in the lower chambers). Because mexiletine itself can trigger arrhythmias, it is not recommended for minor or symptom-free irregular heartbeats.

Treatment is started in a hospital setting so heart rhythm can be monitored closely. Antiarrhythmic drugs, including mexiletine, have not been shown to improve overall survival in patients with ventricular arrhythmias.
