Siponimod falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the connection worth knowing about. Your body relies on a liver enzyme called CYP2C9 to help clear Siponimod. A compound in Ceylon Leadwort, plumbagin, has been shown to slow that same enzyme down. In theory, if the enzyme works slower, Siponimod could build up higher than expected in your system.

For Siponimod, more drug in the body could mean a stronger effect on your heart rate and blood pressure, plus a bigger drop in immune cell activity (raising infection risk). I want to be honest, though: this is based only on lab testing, not real people, so it's a theoretical concern rather than a proven one. Since Siponimod dosing is already carefully tied to your genetics, please check with your pharmacist or doctor before combining the two.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Your body clears Siponimod (Mayzent) partly through a liver enzyme called CYP3A4. Ceylon Leadwort contains plumbagin, which in lab dishes has slowed this same enzyme down. If that happened in real life, Siponimod might be broken down more slowly and build up in your blood, which could mean stronger effects and more side effects, like a slower heart rate, low white blood cell counts, headache, dizziness, higher blood pressure, or liver enzyme changes.

Now, the honest part: this is only test-tube evidence, and it has never been reported in actual people, so the real-world risk is uncertain. Still, since Siponimod needs careful dosing, please run this by your pharmacist or neurologist before combining the two rather than stopping anything on your own.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the gist for Siponimod (Mayzent). This medication works by dialing down certain immune cells so they cause less nerve damage in MS. The concern with Ceylon Leadwort is that its main compound, plumbagin, has shown its own immune-dampening effects in lab and animal studies. In theory, layering it on top of Siponimod could push your immune system down further than intended, which may raise your risk of infections.

I want to be honest about the evidence though: this is based only on test-tube and animal research, not real-world human cases, so it's more of a caution than a proven problem. Still, since Siponimod already affects your immune system, please check with your pharmacist or doctor before adding this supplement rather than starting or stopping anything on your own.

What Is Siponimod Used For?

Brand name: Mayzent

Mayzent (siponimod) is approved for adults with relapsing forms of multiple sclerosis (MS). This includes three specific types: clinically isolated syndrome (a first episode of MS-like symptoms), relapsing-remitting MS (periods of attacks followed by recovery), and active secondary progressive MS (a phase where the disease gradually worsens but still has active relapses).

It is taken by mouth once daily and is intended as a long-term treatment to reduce relapses and slow disease progression.
