Selumetinib Sulfate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist for Koselugo (selumetinib). Your child's body clears this medicine using a liver enzyme called CYP3A4. A compound in Ceylon Leadwort called plumbagin has been shown in lab dish studies to slow that same enzyme down. If it does the same thing in people, selumetinib could build up to higher levels than intended, which may mean more side effects like nausea, diarrhea, vomiting, skin rash, mouth sores, or tiredness. Heart and eye effects are also watched for with this drug.

The honest truth is that this is theoretical right now, based only on test-tube work, and it hasn't been seen in real patients. Still, selumetinib is a serious cancer-type medicine, so please run any Ceylon Leadwort use by your child's oncology team or pharmacist first.

Selumetinib Sulfate also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's something to keep in mind. The interaction on file is really about blood thinners (anticoagulants and antiplatelet drugs), and selumetinib (Koselugo) doesn't fit neatly into that group. So the specific bleeding concern in our database may not apply to selumetinib the way it would to a drug like warfarin or aspirin.

That said, Ceylon Leadwort's bleeding effect is based only on animal studies, not people, so the evidence is weak. The bigger reason to be cautious is that Koselugo already carries its own risks, and adding an unstudied herb on top of a serious cancer-related medication isn't worth the gamble. Please run Ceylon Leadwort past your child's oncology team or pharmacist before using it.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

What Is Selumetinib Used For?

Brand name: Koselugo

Koselugo (selumetinib) is approved for adults and children 1 year of age and older who have neurofibromatosis type 1 (NF1) — a genetic condition that causes tumors to grow on nerve tissue — when they also have symptomatic, inoperable plexiform neurofibromas (large, deep nerve tumors that cannot be surgically removed and are causing problems).

Both the Koselugo capsule and Koselugo oral granule formulations are approved for this use. The oral granule form is intended for patients who have difficulty swallowing whole capsules.
