Warfarin falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the gist for warfarin: lab studies show plumbagin (the active compound in Ceylon Leadwort) can slow down an enzyme called CYP1A2. Warfarin is partly cleared by that enzyme, so in theory, if it's blocked, warfarin could linger longer and build up. With a blood thinner, that could push your blood toward being too thin, raising the risk of bruising, nosebleeds, or more serious bleeding.

The important part: this is only seen in test tubes so far, never confirmed in people, so I don't want to alarm you. But warfarin is a finicky drug where even small shifts change your INR. Please loop in your pharmacist or doctor before combining these, and keep up with your regular INR checks.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the gist for warfarin: your liver clears the more active part of it (called S-warfarin) using an enzyme named CYP2C9. In lab dish studies, plumbagin from Ceylon Leadwort appears to slow that same enzyme down. If that happened in your body, warfarin could pile up and thin your blood more than intended. That matters a lot here because warfarin is a narrow-window drug, so even small shifts can tip your INR too high and raise your risk of bleeding (easy bruising, nosebleeds, bleeding gums, or blood in urine or stool).

I'll be honest, this is only a test-tube finding and has never been confirmed in people, so it's theoretical. Still, with warfarin I'd rather you not gamble. Please check with your doctor or pharmacist before combining them.

Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the situation with these two together. In test tube studies, plumbagin (the active compound in Ceylon Leadwort) appears to slow down a liver enzyme called CYP3A4, which is one of the pathways your body uses to clear Warfarin. In theory, blocking that pathway could let Warfarin build up a bit, which would thin your blood more than intended and raise your risk of bruising, nosebleeds, or more serious bleeding.

I want to be honest though: this is based only on lab research, not on real people, so we don't truly know if it matters in the body. That said, Warfarin is a very finicky drug where even small swings can be a big deal, so I wouldn't combine the two without first checking with your pharmacist or doctor and keeping a close eye on your INR.

Warfarin also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the gist: warfarin is already a powerful blood thinner, and Ceylon Leadwort may add to that effect. In animal studies, the plant's root extract made platelets stickier (less able to clump) and lengthened bleeding time. Stacked on top of warfarin, the worry is too much blood thinning, which could show up as easy bruising, nosebleeds, bleeding gums, blood in your urine or stool, or cuts that won't stop oozing.

The honest part: this has only been seen in lab and animal research, not in people, so we can't say for sure it happens. But warfarin is a touchy drug where small shifts matter, so I'd play it safe. Please check with your doctor or pharmacist before combining them, and don't change your warfarin on your own.

Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Warfarin Used For?

Brand name: Warfarin

Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.

In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
