Warfarin Sodium falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the gist for warfarin. The plumbagin in Ceylon Leadwort has been shown to slow down a liver enzyme called CYP1A2, but only in lab dish (in vitro) studies. Warfarin is partly cleared by that enzyme, so in theory blocking it could let warfarin build up a bit and thin your blood more than intended. With a finely-tuned drug like warfarin, even small shifts matter, and too much can mean easy bruising, nosebleeds, or more serious bleeding.

The important caveat: this has never actually been reported in people, so it's a theoretical concern, not a proven one. Still, because warfarin is so dose-sensitive, please check with your pharmacist or doctor before adding this supplement, and don't change your warfarin on your own.

Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the concern with pairing Ceylon Leadwort and warfarin. Your liver uses an enzyme called CYP2C9 to break down the active part of warfarin (the S-warfarin). A compound in Ceylon Leadwort, plumbagin, has been shown in lab dishes to block that enzyme. If it did the same thing in your body, warfarin could clear out more slowly and build up, which means a stronger blood-thinning effect than your dose intends.

With warfarin, that matters a lot, because it's a narrow-margin drug. Too much can lead to easy bruising, nosebleeds, or more serious bleeding. The good news: this is only seen in lab studies, not in real people yet. Still, warfarin is too touchy to gamble with, so please check with your pharmacist or doctor before adding this supplement.

Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the gist: warfarin already lowers your blood's ability to clot, and Ceylon Leadwort may nudge things in the same direction. In animal studies, the root extract made platelets less sticky and lengthened bleeding time. Stack that on top of warfarin and you could, in theory, see more bruising, nosebleeds, bleeding gums, or blood in the urine or stool.

I want to be honest, though: this hasn't actually been shown in people yet, so it's more of a caution than a proven problem. Still, warfarin is one of those drugs where small shifts can matter a lot, so I wouldn't add this supplement on your own. Please run it past your pharmacist or doctor first, especially since they may want to check your INR.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Warfarin Used For?

Brand names: Coumadin, Panwarfin, Sofarin

Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.

In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
