Oliceridine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Oliceridine (Olinvyk) is an IV opioid given in hospitals for serious short-term pain, and your liver partly clears it using an enzyme called CYP2D6. A compound in Ceylon Leadwort called plumbagin has been shown to slow that same enzyme, but only in lab dish studies so far, not in actual people. If it did slow CYP2D6 in your body, more oliceridine could linger and its effects could run stronger, meaning more drowsiness, nausea, low blood pressure, and importantly slowed breathing.

Since oliceridine works as-is and is not a prodrug, the worry here is too much effect, not too little. Honestly, this is theoretical and most people get oliceridine while closely monitored in a hospital. Still, mention any supplements to your care team so they can keep an eye on you.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Oliceridine (Olinvyk) is a strong IV opioid your body partly clears using a liver enzyme called CYP3A4. Plumbagin, a compound in Ceylon Leadwort, has been shown to slow down that same enzyme, but only in lab dish studies so far. This has not been seen in real people, so think of it as a theoretical heads-up rather than a proven problem.

In theory, if that enzyme is slowed, oliceridine could clear more slowly and build up, which might mean stronger opioid effects like heavy drowsiness, slowed or shallow breathing, low blood pressure, and nausea. Since oliceridine is given in a monitored setting, your care team controls the dose. Still, please mention any Ceylon Leadwort use to your doctor or pharmacist beforehand.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

What Is Oliceridine Injection Used For?

Brand name: Olinvyk

Olinvyk (oliceridine) injection is approved for adults to manage acute pain that is severe enough to require an intravenous opioid analgesic. It is used only when alternative treatments are ineffective, not tolerated, or otherwise unable to provide adequate pain control.

Because of serious risks including addiction, misuse, overdose, and death, Olinvyk is reserved for situations where other pain management options are not sufficient. It is not intended for routine or chronic pain management.
