Why this applies to Sirolimus

Sirolimus falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the connection worth knowing: your body clears sirolimus mainly through a liver enzyme called CYP3A4. In lab studies, plumbagin (the active compound in Ceylon Leadwort) appears to slow that same enzyme down. If it does the same thing in people, your sirolimus could clear more slowly and build up to higher levels than your transplant team intended.

That matters because sirolimus has a narrow safe range, and even small changes can tip the balance. Higher levels can mean more side effects like mouth sores, swelling, high cholesterol, low blood counts, or kidney strain. I'll be honest though, this is only seen in test-tube research so far, never confirmed in humans. Please don't add Ceylon Leadwort without first checking with your transplant doctor or pharmacist.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the gist for anyone on Sirolimus (Rapamune) after a kidney transplant. This medication works by tamping down your immune system so your body doesn't attack the new organ. Ceylon Leadwort contains a compound called plumbagin, which in lab and animal studies has shown its own immune-affecting activity. The concern on file is theoretical: adding it on top of Sirolimus could nudge how your immune system behaves in unpredictable ways.

I want to be straight with you, this is based on test-tube and animal data, not real patients, so we don't have proof it causes problems in people. But Sirolimus is a high-stakes drug where even small shifts matter, and transplant rejection is serious. Please don't add Ceylon Leadwort without first checking with your transplant team or pharmacist.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Sirolimus Used For?

Brand name: Rapamune

Sirolimus oral solution and tablets are used to help prevent organ rejection in kidney (renal) transplant patients aged 13 and older. They are also used to treat lymphangioleiomyomatosis (LAM) — a rare lung disease in which abnormal muscle-like cells invade lung tissue.

Fyarro (sirolimus, intravenous injection) is used to treat adults with locally advanced, unresectable, or metastatic malignant PEComa — a rare tumor that grows in soft tissue or organs. Hyftor (sirolimus topical gel) is used to treat facial angiofibroma (small benign skin growths on the face) associated with tuberous sclerosis in adults and children aged 6 and older.
