Propafenone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the simple version. Your body relies on a liver enzyme called CYP2D6 to clear propafenone. A compound in Ceylon Leadwort called plumbagin has been shown to slow that same enzyme down in lab testing. If it does the same thing inside a person, propafenone could build up higher than expected.

Propafenone is an antiarrhythmic with a fairly narrow safety window, so higher levels could mean more side effects like dizziness, a slow or unusual heartbeat, blurred vision, or a metallic taste. I want to be honest though: this is only a theoretical concern from test-tube work, not something reported in real patients. Still, because of what propafenone does to your heart rhythm, please run it by your pharmacist or doctor before combining them.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the gist for propafenone (Rythmol). Your liver uses a few enzymes to clear this medicine, and one of them is CYP1A2. In lab dishes, plumbagin from Ceylon Leadwort has been shown to slow CYP1A2 down. If that same effect happened in your body, propafenone could be cleared a little slower and build up higher than expected.

Since propafenone steadies your heart rhythm and has a fairly narrow safe range, higher-than-usual levels could mean more side effects like dizziness, a metallic taste, blurred vision, slow heartbeat, or even new rhythm problems. The honest part: this is only a theoretical concern based on test-tube work, not real patient reports. Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Propafenone

What Is Propafenone Used For?

Brand names: Rythmol, Arythmol

Propafenone is used to help control abnormal heart rhythms. It is prescribed to delay the return of paroxysmal atrial fibrillation or atrial flutter (episodic irregular heartbeats in the upper chambers) and paroxysmal supraventricular tachycardia (PSVT, sudden fast heartbeats starting above the heart's lower chambers) — but only in people who do not have structural heart disease.

It is also used to treat life-threatening ventricular arrhythmias (dangerous fast rhythms starting in the heart's lower chambers). Because of serious safety concerns, it is not recommended for minor or non-life-threatening heart rhythm problems.
