Why this applies to Ropivacaine

Ropivacaine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Ropivacaine is a local anesthetic, the kind your provider injects to numb an area for surgery, labor, or pain control. Your liver clears it largely through an enzyme called CYP1A2. Here's the theory: a compound in Ceylon Leadwort called plumbagin can slow that same enzyme down. If it does, ropivacaine could clear more slowly and build up to higher levels than expected.

With more ropivacaine hanging around, you might see effects like numbness or tingling around the mouth, ringing in the ears, dizziness, or in rare cases heart rhythm or seizure problems. I want to be honest, though: this is based only on lab studies, not real people, so the risk is uncertain. Since ropivacaine is given in a clinical setting, just tell your anesthesia team and pharmacist about any supplements first.
