Why this applies to Rivaroxaban

Rivaroxaban falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the connection worth knowing about. Your body uses an enzyme called CYP3A4 to help clear rivaroxaban (Xarelto) out of your system. In lab tests, plumbagin (a compound in Ceylon Leadwort) appears to slow that enzyme down. If that happened in a person, rivaroxaban could clear more slowly and build up, which for a blood thinner means a higher chance of bruising and bleeding, things like nosebleeds, bleeding gums, or blood in the urine or stool.

The important caveat: this is only seen in test-tube studies so far, never confirmed in real people, so it stays theoretical. Still, since rivaroxaban already carries bleeding risk, I'd play it safe. Please check with your pharmacist or doctor before pairing the two, and don't change your Xarelto on your own.

Rivaroxaban also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the gist: Rivaroxaban (Xarelto) is already a blood thinner that lowers your blood's ability to clot. Ceylon Leadwort seems to push in the same direction. In animal studies, its root extract made platelets less sticky and made bleeding last longer. Stack that on top of Xarelto and the worry is simply too much thinning.

What might that look like for you? Easy bruising, nosebleeds, bleeding gums, or bleeding that's slow to stop after a cut. More serious signs would be blood in your urine or stool, or unusually heavy bleeding. To be fair, this is based on lab and animal work, not real human studies, so the risk is more theoretical than proven. Still, with a blood thinner I'd play it safe. Please check with your pharmacist or doctor before combining the two.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Rivaroxaban

What Is Rivaroxaban Used For?

Brand name: Xarelto

Rivaroxaban tablets are used to reduce the risk of stroke in adults with nonvalvular atrial fibrillation (an irregular heart rhythm), treat deep vein thrombosis (DVT, a blood clot in a vein) and pulmonary embolism (PE, a blood clot in the lungs), and lower the chances of these clots coming back. They also help prevent clots after hip or knee replacement surgery, protect hospitalized patients at risk for clots, and — in combination with aspirin — reduce the risk of major cardiovascular events in adults with coronary artery disease (CAD) or peripheral artery disease (PAD).

Rivaroxaban tablets are also approved for treating VTE (venous thromboembolism, blood clots in veins) and preventing recurrence in pediatric patients from birth to under 18 years, and for preventing clots in children aged 2 and older with congenital heart disease after the Fontan procedure. Rivaroxaban for oral suspension (available as Rivaroxaban Granule and Xarelto oral suspension) is specifically indicated for these two pediatric uses.
