These two interact in 4 distinct ways. Each one is explained separately below.
Why Are There 4 Interactions?
Ceylon Leadwort interacts with whole categories of medication. Acetaminophen, Hydrocodone falls into 4 of those categories, so it can interact with Ceylon Leadwort in 4 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 4Why this applies to Acetaminophen, Hydrocodone
Acetaminophen, Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this combo: the hydrocodone in your Vicodin or Norco relies on a liver enzyme called CYP2D6 to turn into hydromorphone, which is a big part of how it eases pain. Ceylon Leadwort contains plumbagin, which has slowed down CYP2D6 in lab studies. If that happened in your body, you'd convert less hydrocodone into its stronger active form, so you might get weaker pain relief than expected.
I want to be straight with you, though: this is only test-tube data, never confirmed in real people, so it's theoretical. The acetaminophen part isn't affected here. Before adding this herb, check with your pharmacist or doctor, and never adjust your pain medicine on your own.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
2 of 4Acetaminophen, Hydrocodone also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with Vicodin or Norco (acetaminophen and hydrocodone) and Ceylon Leadwort. A compound in the plant called plumbagin can slow down a liver enzyme known as CYP1A2 in lab studies. The worry is that blocking this enzyme could let certain medicines build up in your body.
For this combo, I'd put this fairly low on the worry list. Hydrocodone is mainly handled by different enzymes (CYP3A4 and CYP2D6), and acetaminophen isn't a major CYP1A2 drug either, so this particular pathway isn't the main one for either ingredient. On top of that, the evidence is only test-tube data, never actually seen in people. Still, both drugs can cause drowsiness, slowed breathing, and liver stress in larger amounts, so it's smart to run any Ceylon Leadwort use past your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
3 of 4Acetaminophen, Hydrocodone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the practical angle for your Vicodin or Norco (hydrocodone with acetaminophen). The hydrocodone part is actually a prodrug of sorts: your liver uses an enzyme called CYP3A4 to process it, and some of that pathway turns it into a more active painkiller. Plumbagin, a compound in Ceylon Leadwort, has slowed this enzyme down in lab dishes. In theory, that could shift how much active drug builds up, possibly leading to more sedation, drowsiness, slowed breathing, constipation, or nausea.
Important context: this is only seen in test tube research, not in real people, so it's a maybe, not a sure thing. Still, opioids are nothing to gamble with. Please check with your pharmacist or doctor before combining the two.

CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES
4 of 4Acetaminophen, Hydrocodone also falls under CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the interesting twist with this combo. The acetaminophen part of your Vicodin or Norco is partly broken down by an enzyme called CYP2E1. Normally a small slice of acetaminophen gets turned by that enzyme into a harsh byproduct that's tough on the liver. Plumbagin, the active compound in Ceylon Leadwort, actually blocks that enzyme in lab testing, so in theory it could shift how acetaminophen is processed. The honest truth is this has only been seen in test tubes, not in real people, so I wouldn't lose sleep over it.
Still, I'd mention it to your pharmacist, especially since acetaminophen is already hard on the liver in larger amounts. Occasional culinary use is unlikely to matter, but concentrated supplements are a different story. Don't change your pain medicine on your own.

From our pharmacist’s overview on the Acetaminophen, Hydrocodone page
“Someone taking hydrocodone with acetaminophen (Vicodin, Norco) for post-surgical or dental pain often reaches for a sleep aid, kava, or a glass of wine at night, and that is exactly where the risk sits. This combination calls for genuine vigilance.”

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain.
