Acetaminophen, Diphenhydramine, Pseudoephedrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the honest picture with this combo cold-and-allergy product. A compound in Ceylon Leadwort called plumbagin can slow down a liver enzyme known as CYP1A2, but only in lab dish studies so far. Nothing like this has actually been reported in people, so I wouldn't lose sleep over it.

Of the three ingredients here, none lean heavily on CYP1A2 for clearance, so a meaningful change in their levels is unlikely. If anything did build up, you might notice more of the usual effects: extra drowsiness or dry mouth from the diphenhydramine, or jitteriness from the pseudoephedrine. Because this is theoretical, just give your pharmacist a heads-up that you're using Ceylon Leadwort, and avoid concentrated doses to be safe.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2E1 (CYP2E1) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2E1. Some drugs metabolized by CYP2E1 include acetaminophen, chlorzoxazone (Parafon Forte), ethanol, theophylline, and anesthetics such as enflurane (Ethrane), halothane (Fluothane), isoflurane (Forane), methoxyflurane (Penthrane).

Here's the interesting part of this combo: of the three ingredients in your cold and allergy product, it's the acetaminophen (Tylenol) piece that matters here. One enzyme, called CYP2E1, helps process acetaminophen. In lab studies, plumbagin from Ceylon Leadwort slowed that enzyme down. In theory, that could let a little more acetaminophen linger in your system.

I want to be honest with you though, this has only been seen in test tubes, never in real people, so I wouldn't lose sleep over it. The bigger everyday rule still stands: don't double up on acetaminophen from multiple products, and keep your daily total in a safe range to protect your liver. The diphenhydramine and pseudoephedrine aren't part of this particular concern. If you're using Ceylon Leadwort regularly, just give your pharmacist a heads-up so we can check your total acetaminophen load.
