Amitriptyline, Perphenazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

This combination pill pairs amitriptyline (an antidepressant) with perphenazine (an antipsychotic). Both rely partly on an enzyme called CYP2D6 to be cleared from your body. Plumbagin, a compound in Ceylon Leadwort, has been shown to slow down that enzyme, but only in lab dish studies so far, never in real people. The concern is theoretical: if CYP2D6 is blocked, these medicines could build up a bit.

If levels did rise, you might notice more of their usual side effects, like heavy drowsiness, dry mouth, constipation, blurry vision, dizziness on standing, or a faster heartbeat. Since this is only based on lab data, I wouldn't panic, but I would run it by your pharmacist or doctor before adding this supplement.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

This combination pill pairs amitriptyline (an older antidepressant) with perphenazine (an antipsychotic). Amitriptyline is partly broken down by a liver enzyme called CYP1A2. In lab dish studies, plumbagin from Ceylon Leadwort slowed that same enzyme down. In theory, that could let amitriptyline build up a bit more than usual.

If that happened, you might notice stronger amitriptyline-type effects like drowsiness, dry mouth, constipation, blurred vision, dizziness when standing, or a faster heartbeat. I want to be honest though: this is only a test-tube finding, never reported in real people, so it may not matter much in practice. Still, since amitriptyline can affect heart rhythm, please check with your pharmacist or doctor before adding this supplement rather than changing anything on your own.

Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the situation with the combo pill that pairs amitriptyline (an antidepressant) with perphenazine (an antipsychotic). A compound in Ceylon Leadwort called plumbagin has, in lab dish studies only, slowed down an enzyme called CYP2C9. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.
