Red yeast rice contains varying levels of the statin drug lovastatin, which is metabolized by CYP3A4. Combining red yeast rice with CYP3A4 inhibitors might increase serum levels of lovastatin from red yeast rice.

Here's the thing to know about pairing these two. Aprepitant (Emend) gently blocks a liver enzyme called CYP3A4, and that's the very same enzyme that breaks down the natural lovastatin found in red yeast rice. So when you take them together, your body may clear the red yeast rice's statin more slowly, letting it build up a bit higher than usual.

With more statin floating around, the main worry is muscle aches, weakness, or tenderness, and rarely a more serious muscle problem. I'll be honest, this is a theoretical concern based on how these drugs work, not proven in studies, and aprepitant is usually only taken for a few days around chemo. Still, please run it by your oncology pharmacist or doctor before combining them.

Red yeast rice contains monacolin K, which is chemically identical to the prescription statin lovastatin, and it can lower LDL ('bad') cholesterol. About Aprepitant

What Is Aprepitant Used For?

Brand names: Emend, Cinvanti

Aprepitant is used to prevent nausea and vomiting — not to treat it once it has started. It is always used alongside other anti-nausea medicines, not on its own.

EMEND capsules (oral) are approved for adults and children 12 and older to prevent nausea from highly or moderately emetogenic (nausea-causing) chemotherapy. EMEND for oral suspension extends that same use down to children as young as 6 months. CINVANTI (IV injection) is approved for adults only, for chemotherapy-related nausea. APONVIE (IV emulsion) is approved for adults to prevent postoperative nausea and vomiting (PONV) — nausea after surgery. Aprepitant capsules (generic) are approved for adults and children 12 and older for chemotherapy-related nausea, and also for adults to prevent PONV.
