Prednisone falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Red Yeast Rice interacts with.
Red yeast rice contains varying levels of the statin drug lovastatin, which is metabolized by CYP3A4. Combining red yeast rice with CYP3A4 inhibitors might increase serum levels of lovastatin from red yeast rice.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the angle that matters with this pair. Prednisone can act as a mild blocker of the liver enzyme CYP3A4, and that same enzyme is what breaks down the lovastatin naturally found in red yeast rice. So in theory, taking them together could let the statin part of red yeast rice build up a bit higher than usual in your blood.
If that happens, you'd be watching for the typical statin-type side effects: muscle aches, soreness, or weakness, and rarely, liver irritation. The good news is this is a theoretical concern based on how the drugs work, not something proven in studies, so don't panic. Still, red yeast rice doses vary a lot, so it's worth running this combo past your pharmacist or doctor first.
