Nitrofurantoin falls under HEPATOTOXIC DRUGS — the group Red Yeast Rice interacts with.

Theoretically, concomitant use might increase the risk of liver damage.
Red yeast rice contains varying levels of the drug lovastatin. Lovastatin can cause liver damage in some people. Some clinical research suggests that supplements containing red yeast rice might increase liver enzyme levels in some, but not all, participants. Cases of acute hepatitis have been associated with red yeast rice. Combining it with hepatotoxic drugs might further increase this risk.

Here's the simple version: both red yeast rice and nitrofurantoin can, on rare occasions, be tough on the liver. Red yeast rice naturally contains lovastatin, a real statin that can nudge liver enzymes up in some people. Nitrofurantoin (Macrobid, Macrodantin) is also linked to occasional liver irritation. Stacking two things that lean on the same organ is the concern here.

I want to be honest with you though: this is a theoretical caution based on how these products behave, not proven from studies of the two together. Your nitrofurantoin course is usually short, which keeps the risk low. Still, watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before combining them.

Red yeast rice contains monacolin K, which is chemically identical to the prescription statin lovastatin, and it can lower LDL ('bad') cholesterol. Because of this, it carries the same risks as statin drugs, and the amount of active ingredient varies widely between products. Always talk to your pharmacist or doctor before using it, especially if you have liver problems or take other medicines.

About Nitrofurantoin

What Is Nitrofurantoin Used For?

Brand names: Macrodantin, Furadantin, Furatoin, Macrobid

Nitrofurantoin is used to treat urinary tract infections (UTIs) caused by certain susceptible bacteria. The capsule forms (including Macrodantin) are indicated for UTIs due to susceptible strains of Escherichia coli, enterococci, Staphylococcus aureus, and certain strains of Klebsiella and Enterobacter species. Nitrofurantoin oral suspension (Furadantin) is indicated for adults and children 1 month of age and older for the same types of UTIs.

Nitrofurantoin is not used for kidney infections (pyelonephritis) or infections of the tissue around the kidney (perinephric abscesses). Because it works primarily in the urine and does not spread widely through body tissues, it may not fully clear the infection in some patients, and the UTI may return after treatment.
