Red yeast rice contains varying levels of the drug lovastatin. Lovastatin can cause liver damage in some people. Some clinical research suggests that supplements containing red yeast rice might increase liver enzyme levels in some, but not all, participants. Cases of acute hepatitis have been associated with red yeast rice. Combining it with hepatotoxic drugs might further increase this risk.

Here's the simple version: both rifampin and red yeast rice can be tough on your liver. Rifampin itself can sometimes raise liver enzymes or, less often, cause real liver inflammation. Red yeast rice acts like a low-dose statin (it actually contains lovastatin), and statins carry their own small risk of liver irritation. Stacking two liver-stressing products together might add up.

I'll be honest, this concern is mostly theoretical, based on how these things work rather than strong studies. Still, since you're already on rifampin, I'd run the red yeast rice by your pharmacist or doctor first. And watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and report those right away.

Red yeast rice contains monacolin K, which is chemically identical to the prescription statin lovastatin, and it can lower LDL ('bad') cholesterol. Because of this, it carries the same risks as statin drugs, and the amount of active ingredient varies widely between products. What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
