Red yeast rice contains varying levels of the statin drug lovastatin, which is metabolized by CYP3A4. Combining red yeast rice with CYP3A4 inhibitors might increase serum levels of lovastatin from red yeast rice.

Here's the thing to know about pairing these two. Everolimus is a CYP3A4 inhibitor, meaning it can slow down that liver enzyme. Red yeast rice contains lovastatin, a statin that gets broken down by that very same enzyme. So when you take them together, the lovastatin from your red yeast rice may not clear out as fast and can build up in your blood.

Higher statin levels raise the odds of statin side effects, mainly muscle aches, weakness, and (rarely) more serious muscle breakdown, plus possible liver strain. This is a theoretical concern based on how the drugs work, not hard proof, and red yeast rice products vary a lot in strength. Since you're already on a serious cancer medicine, please loop in your pharmacist or oncologist before adding it.

What Is Everolimus Used For?

Brand names: Certican, Afinitor, Zortress, Afinitor Disperz

Everolimus is used to treat several serious conditions. In cancer, it is prescribed for advanced hormone receptor-positive, HER2-negative breast cancer (in postmenopausal women), certain neuroendocrine tumors of the pancreas, GI tract, or lung, and advanced renal cell carcinoma (kidney cancer) when other treatments have stopped working.

It is also used for people with tuberous sclerosis complex (TSC) — a genetic condition that causes benign tumors to grow throughout the body. In TSC, it treats kidney tumors called angiomyolipomas, brain tumors called SEGA, and seizures. In kidney transplant patients, it helps prevent the body from rejecting the new organ.
