Red yeast rice contains varying levels of the drug lovastatin. Lovastatin can cause liver damage in some people. Some clinical research suggests that supplements containing red yeast rice might increase liver enzyme levels in some, but not all, participants. Cases of acute hepatitis have been associated with red yeast rice. Combining it with hepatotoxic drugs might further increase this risk.

Here's the gist: red yeast rice contains the same active ingredient as the statin lovastatin, and statins occasionally stress the liver. Cyclosporine can also be tough on the liver in some people. So putting the two together might add up to a higher chance of liver irritation. I want to be honest, this concern is mostly theoretical, based on how these substances behave, not on strong studies in people taking both.

There's a second wrinkle worth a mention. Cyclosporine actually raises blood levels of statins like lovastatin, which can bump up the risk of muscle pain or weakness. Cyclosporine is also a narrow-margin drug where small changes matter. Please don't start or stop anything on your own. Loop in your pharmacist or transplant team first.

Here's the key thing to know: cyclosporine slows down an enzyme in your liver called CYP3A4, and that same enzyme is what breaks down the lovastatin naturally found in red yeast rice. So if you take them together, the statin from the red yeast rice can build up to higher levels than expected. That matters because more statin in your system means a greater chance of muscle aches and weakness, and in rare cases serious muscle breakdown that can stress the kidneys (something transplant patients especially want to avoid).

To be fair, this concern is theoretical, based on how these compounds behave rather than hard studies. Red yeast rice products also vary a lot in strength. Still, with a medicine as important as cyclosporine, please check with your pharmacist or doctor before combining them.

Theoretically, taking red yeast rice in combination with cyclosporine might increase the risk of myopathy.
Red yeast rice contains varying levels of the statin drug lovastatin. Cyclosporine has been reported to increase plasma levels of lovastatin by 5- to 20-fold, resulting in reports of myopathy and rhabdomyolysis.

Here's the thing about red yeast rice: it naturally contains lovastatin, the very same statin you'd get by prescription. When you pair that with cyclosporine, you run into a real problem. Cyclosporine slows down how your body clears statins like lovastatin, and studies have shown it can push lovastatin blood levels up by 5 to 20 times. That's a big jump, and it raises the risk of muscle problems.

What that looks like in real life is muscle pain, weakness, or tenderness, and in rare cases a serious breakdown of muscle tissue (rhabdomyolysis) that can hurt the kidneys. Since red yeast rice products vary a lot in how much lovastatin they contain, you can't really predict the dose. The evidence here is mostly theoretical and anecdotal, but given the stakes, please talk to your pharmacist or doctor before combining these.

Cyclosporine is prescribed to help prevent the body from rejecting a transplanted kidney, liver, or heart. It is often used alongside other immunosuppressive medicines as part of a full transplant care plan.

It is also used for adults with severe, active rheumatoid arthritis that hasn't responded well enough to methotrexate, and for adults with severe plaque psoriasis who haven't had success with other standard treatments.
