Red Yeast Rice and Erythromycin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS

Why this applies to Erythromycin

Erythromycin falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Red Yeast Rice interacts with.

Theoretically, drugs that inhibit the CYP3A4 enzymes might increase levels of lovastatin from red yeast rice.
Red yeast rice contains varying levels of the statin drug lovastatin, which is metabolized by CYP3A4. Combining red yeast rice with CYP3A4 inhibitors might increase serum levels of lovastatin from red yeast rice.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know if you take red yeast rice while on a course of erythromycin. Red yeast rice naturally contains lovastatin, the same active ingredient found in the prescription statin. Your liver clears that lovastatin using an enzyme called CYP3A4, and erythromycin is a fairly strong blocker of that same enzyme. So while you're taking the antibiotic, the statin from your supplement can build up higher than usual.

When statin levels climb, the main worry is muscle trouble: aches, soreness, or weakness, and in rare cases more serious muscle breakdown. This is mostly theoretical and depends on how much lovastatin your particular product contains (it varies a lot). Erythromycin courses are usually short, but it's still worth asking your pharmacist whether to pause the red yeast rice while you finish the antibiotic.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

From our pharmacist’s overview on the Erythromycin page

“Erythromycin calls for more supplement caution than most antibiotics, and the reason is one liver enzyme, CYP3A4. Erythromycin both blocks that enzyme and is broken down by it, so grapefruit can push erythromycin levels up while St.”

About Red Yeast Rice

What is Red Yeast Rice?

Red Yeast Rice
  • High cholesterol
  • Heart health
  • Metabolic syndrome
  • Statin alternative

Red yeast rice contains monacolin K, which is chemically identical to the prescription statin lovastatin, and it can lower LDL ('bad') cholesterol. Because of this, it carries the same risks as statin drugs, and the amount of active ingredient varies widely between products. Always talk to your pharmacist or doctor before using it, especially if you have liver problems or take other medicines.
