Ciprofloxacin falls under HEPATOTOXIC DRUGS — the group Red Yeast Rice interacts with.
Red yeast rice contains varying levels of the drug lovastatin. Lovastatin can cause liver damage in some people. Some clinical research suggests that supplements containing red yeast rice might increase liver enzyme levels in some, but not all, participants. Cases of acute hepatitis have been associated with red yeast rice. Combining it with hepatotoxic drugs might further increase this risk.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: red yeast rice naturally contains lovastatin, a statin that can occasionally stress your liver and bump up liver enzymes. Ciprofloxacin, while uncommon, can also irritate the liver in some people. Stacking two things that each carry a small liver risk is the concern here. To be clear, this is a theoretical caution based on how these substances behave, not proof that taking them together will harm you.
If you do take both, watch for warning signs like yellowing of the skin or eyes, dark urine, belly pain on your right side, unusual tiredness, or nausea. Most cipro courses are short, which lowers the worry, but it's worth a quick chat with your pharmacist or doctor before combining them, especially if you have any liver issues.

CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS

Red yeast rice contains varying levels of the statin drug lovastatin, which is metabolized by CYP3A4. Combining red yeast rice with CYP3A4 inhibitors might increase serum levels of lovastatin from red yeast rice.

Here's the thing to know if you take red yeast rice and get prescribed ciprofloxacin (Cipro). The active part of red yeast rice is basically the statin lovastatin, and your liver clears it using an enzyme called CYP3A4. Ciprofloxacin can mildly slow that same enzyme. In theory, that could let the statin part build up a little higher than usual.
If statin levels climb too much, the main worry is muscle aches, soreness, or weakness, and rarely more serious muscle breakdown. To be clear, this is a theoretical concern based on how the drugs work, not strong proof. Cipro is also usually a short course. Still, it's worth a quick chat with your pharmacist or doctor, especially since red yeast rice doses vary a lot.
