Aprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Yohimbe interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of yohimbine, a constituent of yohimbe bark.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 might increase the levels and adverse effects of yohimbine.

Here's the thing to know if you're taking aprepitant (Emend) alongside a yohimbe supplement. Aprepitant slows down a liver enzyme called CYP3A4, which happens to be the same enzyme your body uses to break down yohimbine, the active compound in yohimbe. So when these two are combined, yohimbine may clear out of your system more slowly and build up to higher levels than expected.

In plain terms, that could make yohimbe's stimulant-type effects stronger, things like a racing heartbeat, jumpiness, anxiety, and elevated blood pressure. This is a theoretical concern based on how the drugs are processed, not something proven in studies, so I don't want to alarm you. Still, yohimbe can be risky on its own. Please check with your pharmacist or doctor before pairing them.

Here's the situation with these two. Aprepitant (Emend) is cleared from your body by a liver enzyme called CYP3A4. Lab studies suggest yohimbe may speed that enzyme up. In theory, a faster enzyme could break down aprepitant more quickly, leaving lower levels in your system. That could mean aprepitant doesn't control your chemo-related nausea and vomiting as well as it should.

I want to be honest about the strength of this: it's based only on test-tube research, not real patient studies, so it's labeled minor and theoretical. Still, since aprepitant is part of preventing serious chemo nausea, and yohimbe itself can spike your blood pressure and heart rate, please check with your oncology team or pharmacist before combining them.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Aprepitant

What Is Aprepitant Used For?

Brand names: Emend, Cinvanti

Aprepitant is used to prevent nausea and vomiting — not to treat it once it has started. It is always used alongside other anti-nausea medicines, not on its own.

EMEND capsules (oral) are approved for adults and children 12 and older to prevent nausea from highly or moderately emetogenic (nausea-causing) chemotherapy. EMEND for oral suspension extends that same use down to children as young as 6 months. CINVANTI (IV injection) is approved for adults only, for chemotherapy-related nausea. APONVIE (IV emulsion) is approved for adults to prevent postoperative nausea and vomiting (PONV) — nausea after surgery. Aprepitant capsules (generic) are approved for adults and children 12 and older for chemotherapy-related nausea, and also for adults to prevent PONV.
