Aprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Zedoary interacts with.

Theoretically, zedoary might increase levels of drugs metabolized by CYP3A4.
In-vitro research shows that a methanol extract of zedoary strongly inhibits the CYP3A4 metabolism of the tyrosine kinase inhibitors lapatinib and sorafenib, and to a lesser extent, gefitinib.

Here's the gist with aprepitant (Emend, Cinvanti). Your body relies on a liver enzyme called CYP3A4 to clear this drug from your system. Lab studies suggest zedoary may slow that same enzyme down. If it does, aprepitant could linger longer and build up, which may bump up its usual side effects like fatigue, hiccups, headache, dizziness, or mild stomach upset.

I want to be honest about the strength of this concern: it's theoretical, based only on test-tube work, not on real people taking these two together. Sprinkling zedoary in food likely matters far less than a concentrated supplement dose. Since aprepitant is part of important anti-nausea care during chemo, just run it past your oncology pharmacist or doctor before adding zedoary.

About Aprepitant

What Is Aprepitant Used For?

Brand names: Emend, Cinvanti

Aprepitant is used to prevent nausea and vomiting — not to treat it once it has started. It is always used alongside other anti-nausea medicines, not on its own.

EMEND capsules (oral) are approved for adults and children 12 and older to prevent nausea from highly or moderately emetogenic (nausea-causing) chemotherapy. EMEND for oral suspension extends that same use down to children as young as 6 months. CINVANTI (IV injection) is approved for adults only, for chemotherapy-related nausea. APONVIE (IV emulsion) is approved for adults to prevent postoperative nausea and vomiting (PONV) — nausea after surgery. Aprepitant capsules (generic) are approved for adults and children 12 and older for chemotherapy-related nausea, and also for adults to prevent PONV.
