Zedoary and Doxorubicin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Doxorubicin

Doxorubicin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Zedoary interacts with.

Theoretically, zedoary might increase levels of drugs metabolized by CYP3A4.
In-vitro research shows that a methanol extract of zedoary strongly inhibits the CYP3A4 metabolism of the tyrosine kinase inhibitors lapatinib and sorafenib, and to a lesser extent, gefitinib.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic idea: your body uses an enzyme called CYP3A4 to help clear doxorubicin. Zedoary, in lab studies, appears to block that enzyme. If it does the same thing in people, doxorubicin could clear more slowly and build up a bit, which might mean more of its usual side effects, things like nausea, mouth sores, low blood counts, and the heart-related effects this drug is known for.

I want to be honest about the strength of this, though. So far it's only theoretical, coming from test-tube work, not from people. Still, because chemotherapy is serious business and the dosing is precise, I wouldn't add zedoary on your own. Please run it by your oncologist or pharmacist first so they can weigh in.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Doxorubicin page

“Doxorubicin (Adriamycin, Doxil) is a chemotherapy drug, and with chemotherapy there is little room for a drug level that drifts up or down, so this list deserves genuine care. Of the 139 supplements and foods on file, only two carry a major rating, but they are common ones: grapefruit, which can raise levels of drugs processed the way…”

About Doxorubicin

What Is Doxorubicin Used For?

Brand names: Doxil, Adriamycin, Rubex, Caelyx, Myocet

Standard (non-liposomal) doxorubicin products, including Adriamycin, are used with other chemotherapy after surgery for women with breast cancer that has spread to nearby lymph nodes. They are also used to treat acute lymphoblastic leukemia, acute myeloblastic leukemia, Hodgkin lymphoma, and non-Hodgkin lymphoma. Other uses include several metastatic (spread) cancers: breast, Wilms’ tumor, neuroblastoma, soft tissue sarcoma, bone sarcoma, ovarian, bladder, thyroid, stomach, and lung (bronchogenic).

Liposomal doxorubicin products, including Doxil, have different uses. They treat ovarian cancer that has progressed after platinum-based chemotherapy, and AIDS-related Kaposi’s sarcoma after other chemotherapy failed or was not tolerated. They also treat multiple myeloma, in combination with bortezomib, in people who have not had bortezomib and have had at least one prior therapy.
