Zedoary and Avatrombopag Maleate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Avatrombopag Maleate

Avatrombopag Maleate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Zedoary interacts with.

Theoretically, zedoary might increase levels of drugs metabolized by CYP3A4.
In-vitro research shows that a methanol extract of zedoary strongly inhibits the CYP3A4 metabolism of the tyrosine kinase inhibitors lapatinib and sorafenib, and to a lesser extent, gefitinib.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Doptelet (avatrombopag) and zedoary. Your body uses a liver enzyme called CYP3A4 to help break down avatrombopag. Zedoary may slow that enzyme down, at least based on test-tube studies. If it does, more of the drug could linger in your system, which in theory could nudge your platelet count higher than intended and raise the risk of clot-related problems.

I want to be honest about the strength of this: it's theoretical, drawn from lab research, not from people taking both together. A dash of zedoary as a cooking spice is very unlikely to matter. Concentrated supplement capsules are the bigger question mark. Since avatrombopag is dosed carefully, run it past your pharmacist or doctor before adding zedoary.

From our pharmacist’s overview on the Avatrombopag Maleate page

“Avatrombopag maleate (Doptelet) exists to raise a low platelet count, so anything that drags its blood level down invites the bleeding risk it was prescribed to prevent. I would handle this list of 54 supplements with real care, and the two rated major come first.”
