Neomycin falls under ANTIBIOTIC DRUGS — the group Lacticaseibacillus Casei interacts with.

Theoretically, taking Lacticaseibacillus casei with antibiotic drugs might decrease the effectiveness of L. casei.
L. casei preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. casei preparations by at least two hours.

Here's the simple version: this interaction runs in one direction. Neomycin is an antibiotic, and antibiotics don't care whether bacteria are good or bad. If you swallow a Lacticaseibacillus casei probiotic at the same time as neomycin, the antibiotic can kill off many of those live, friendly bugs before they ever get to work. So you'd likely get less benefit from your probiotic, not more side effects from the neomycin.

The good news is your neomycin keeps working just fine. To get the most out of both, a common tip is to space them apart by at least two hours. This concern is mostly theoretical, based on how the two work, so check with your pharmacist or doctor before changing anything.
