Why this applies to Tobramycin

Tobramycin falls under AMINOGLYCOSIDE ANTIBIOTICS — the group Sodium Bicarbonate interacts with.

Theoretically, sodium bicarbonate may increase the risk for hypokalemia in patients receiving aminoglycosides.
Orally, use of excessive sodium bicarbonate (such as the intake of "tablespoons" of sodium bicarbonate daily or up to one box of baking soda weekly) has been associated with cases of hypokalemia. Furthermore, when administered intravenously, the most common complication of sodium bicarbonate is hypokalemia. Nephrotoxicity caused by aminoglycosides may lead to increased urinary losses of various electrolytes, including potassium.

Here's the gist with tobramycin and baking soda: both can lower your potassium, so using them together may push that drop a little further. Tobramycin can be hard on the kidneys, which can make you lose potassium in your urine. Large amounts of sodium bicarbonate (think tablespoons daily or close to a box a week) can also drain potassium. Stack those and your level could dip.

That said, this is a theoretical concern based on how these work, not strong proof. A normal antacid dose now and then is unlikely to be the problem. Low potassium can cause muscle cramps, weakness, or an irregular heartbeat, so if you're on tobramycin, check with your pharmacist or doctor before using sodium bicarbonate regularly.

Tobramycin Injection is used to treat serious and life-threatening bacterial infections caused by susceptible bacteria. It is approved for adults and children for infections of the blood (septicemia), lungs and airways, brain and spinal cord (meningitis), abdomen, skin, bones, and urinary tract.

Because antibiotic resistance is a growing concern, Tobramycin Injection should only be used when there is a proven or strongly suspected bacterial infection. Your care team will use culture and lab results to confirm it is the right choice.
