Sodium Phosphate (OTC drug) falls under SODIUM-CONTAINING DRUGS — the group Sodium Bicarbonate interacts with.

Concomitant use of sodium-containing drugs with additional sodium from dietary or supplemental sources may increase the risk of hypernatremia and long-term sodium-related adverse effects.
The Chronic Disease Risk Reduction (CDRR) intake level of 2.3 grams of sodium daily indicates the intake at which it is believed that chronic disease risk increases for the apparently healthy population. Some medications contain high quantities of sodium. When used in conjunction with sodium bicarbonate, the CDRR may be exceeded. Additionally, concomitant use may increase the risk for hypernatremia; this risk is highest in the elderly and people with other risk factors for electrolyte disturbances.

Here's the thing to keep in mind: both of these products pile on sodium. Oral sodium phosphate (like Fleet Phospho-Soda) is a strong laxative often used to clear the bowels, and it already delivers a hefty salt and electrolyte load. Add baking soda (sodium bicarbonate) on top, and you're stacking sodium on sodium.

This is mostly a theoretical concern based on how the two work, not something proven to harm most healthy people. Still, the realistic worry is electrolyte trouble: rising sodium (hypernatremia), dehydration, headache, swelling, or feeling weak and confused, especially in older adults or anyone with heart, kidney, or blood pressure problems. Sodium phosphate can already shift fluids and minerals on its own, so I'd run this combo by your pharmacist or doctor before using them together.
