Arformoterol falls under BETA-ADRENERGIC AGONISTS — the group Sodium Bicarbonate interacts with.

Theoretically, sodium bicarbonate may increase the risk for hypokalemia in patients taking beta-adrenergic agonists.
Orally, use of excessive sodium bicarbonate (such as the intake of "tablespoons" of sodium bicarbonate daily or up to one box of baking soda weekly) has been associated with cases of hypokalemia. Furthermore, the most common adverse effect of intravenous sodium bicarbonate is hypokalemia. Oral, parenteral, or inhaled beta-adrenergic agonists can reduce serum potassium levels, especially during acute use of high doses.

Here's the thing to know about pairing Brovana (arformoterol) with baking soda. Arformoterol is a long-acting beta agonist for COPD, and one of its quieter effects is that it can nudge your potassium levels down a bit, especially early on or at higher doses. Sodium bicarbonate, if you take a lot of it, can do the same thing. So in theory, using both together could lower potassium more than either one alone.

I want to be honest about the evidence here: this is a theoretical concern based on how these two work, not something documented happening often. And it really takes large amounts of baking soda (think tablespoons daily or close to a box a week) to matter. A small occasional pinch for heartburn is unlikely to be a problem. Low potassium can cause muscle cramps, weakness, or an irregular heartbeat, so if you're using bicarbonate regularly, mention it to your pharmacist or doctor.

Arformoterol tartrate inhalation solution is used for the long-term, twice-daily maintenance treatment of bronchoconstriction (narrowed airways) in adults with chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema.

Important: This medication is not intended to treat sudden COPD flare-ups or acute breathing emergencies — it will not work fast enough. It is also not approved for treating asthma.
