Why this applies to Hydrocortisone

Hydrocortisone falls under CORTICOSTEROIDS — the group Sodium Bicarbonate interacts with.

Theoretically, sodium bicarbonate may increase the risk of hypokalemia in patients taking corticosteroids.
Orally, use of excessive sodium bicarbonate (such as the intake of "tablespoons" of sodium bicarbonate daily or up to one box of baking soda weekly) has been associated with cases of hypokalemia. Furthermore, the most common intravenous complication of sodium bicarbonate is hypokalemia. Some glucocorticoids (corticosteroids) can also cause hypokalemia by causing sodium retention, resulting in compensatory renal potassium excretion. It is most common with hydrocortisone, cortisone, and fludrocortisone, followed by prednisone and prednisolone.

Here's the gist with hydrocortisone and baking soda. Both can quietly push your potassium down. Hydrocortisone makes your body hold onto sodium, and to balance that, your kidneys dump more potassium in the urine. Large amounts of sodium bicarbonate can lower potassium too. Stack the two and the effect could add up.

This is a theoretical concern based on how these work, not strong proof, so don't panic. It mostly matters with heavy baking soda use (think tablespoons daily, not one fizzy glass for occasional heartburn). If potassium drops too low you might feel muscle weakness, cramps, fatigue, or a fluttery heartbeat. A normal antacid dose now and then is usually fine. If you take hydrocortisone regularly and use baking soda often, just run it by your pharmacist or doctor first.

Brand names: Cortef, Hydrocortone, Carmol HC cream, Hydrocortisone Iodoquinol Cream, Vytone Cream, Massengil Medicated Towelettes, Dioderm, Efcortelan Cream, Mildison, Alphaderm, Calmurid HC, Efcortelan Ointment, Texacort, Alkindi Sprinkle

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
