Why this applies to Fluticasone

Fluticasone falls under CORTICOSTEROIDS — the group Sodium Bicarbonate interacts with.

Theoretically, sodium bicarbonate may increase the risk of hypokalemia in patients taking corticosteroids.
Orally, use of excessive sodium bicarbonate (such as the intake of "tablespoons" of sodium bicarbonate daily or up to one box of baking soda weekly) has been associated with cases of hypokalemia. Furthermore, the most common intravenous complication of sodium bicarbonate is hypokalemia. Some glucocorticoids (corticosteroids) can also cause hypokalemia by causing sodium retention, resulting in compensatory renal potassium excretion. It is most common with hydrocortisone, cortisone, and fludrocortisone, followed by prednisone and prednisolone.

Here's the good news: this is mostly a theoretical concern, and it really doesn't fit fluticasone well. Fluticasone (whether you're using Flonase in your nose or Flovent in your lungs) acts mainly right where you spray it, so very little gets into your bloodstream. The worry on file is that steroids plus large amounts of baking soda could lower your potassium (hypokalemia), but that effect is tied to oral or IV steroids like prednisone or hydrocortisone, not a low-dose inhaled or nasal product like this one.

To even nudge potassium, you'd be swallowing huge amounts of baking soda, like spoonfuls daily. A normal antacid pinch now and then is very unlikely to matter with fluticasone. Still, if you have heart, kidney, or blood pressure issues, check with your pharmacist before using baking soda regularly.
