Bumetanide falls under LOOP DIURETICS — the group Sodium Bicarbonate interacts with.

Theoretically, sodium bicarbonate may increase the risk of hypokalemia in patients taking loop diuretics.
Loop diuretics increase urinary potassium excretion. Orally, use of excessive sodium bicarbonate (such as the intake of "tablespoons" of sodium bicarbonate daily or up to one box of baking soda weekly) has been associated with cases of hypokalemia. Furthermore, the most common complication of intravenous sodium bicarbonate is hypokalemia.

Here's the gist for bumetanide. As a loop diuretic, it makes your kidneys flush out extra water and salt, but it also pushes potassium out in the process. That's why low potassium (hypokalemia) is one of its more common side effects. The concern here is that large amounts of sodium bicarbonate can do the same thing, so stacking the two could lower your potassium more than expected.

Now, the honest part: this is a theoretical concern based on how these work, not strong real-world data, and it really only showed up with heavy baking soda use (think tablespoons daily). An occasional pinch for heartburn is unlikely to cause trouble. Still, if you notice muscle cramps, weakness, or an odd heartbeat, or if you reach for baking soda regularly, check in with your pharmacist or doctor first.

What Is Bumetanide Used For?

Brand name: Burinex

Bumetanide treats edema — excess fluid that builds up in the body — caused by congestive heart failure, hepatic (liver) disease, and renal (kidney) disease, including a condition called nephrotic syndrome. This applies to the oral tablets (bumetanide and Bumex), the injection (Bumetanide Injection), and the nasal spray.

Enbumyst (bumetanide nasal spray) is specifically approved for these same uses in adults. Enbumyst is not intended for long-term use and should be switched to an oral diuretic as soon as practical.
