Why this applies to Amikacin

Amikacin falls under AMINOGLYCOSIDE ANTIBIOTICS — the group Sodium Bicarbonate interacts with.

Theoretically, sodium bicarbonate may increase the risk for hypokalemia in patients receiving aminoglycosides.
Orally, use of excessive sodium bicarbonate (such as the intake of "tablespoons" of sodium bicarbonate daily or up to one box of baking soda weekly) has been associated with cases of hypokalemia. Furthermore, when administered intravenously, the most common complication of sodium bicarbonate is hypokalemia. Nephrotoxicity caused by aminoglycosides may lead to increased urinary losses of various electrolytes, including potassium.

Here's the gist: this isn't about baking soda changing how amikacin works. Amikacin is an injectable antibiotic your medical team gives directly, so sodium bicarbonate won't make it stronger or weaker. The real concern is potassium. Amikacin can be hard on the kidneys, and that can cause you to lose extra potassium in your urine. Sodium bicarbonate can also pull potassium down on its own.

Stack those two together and your potassium could dip low, which may show up as muscle weakness, cramps, fatigue, or an irregular heartbeat. Keep in mind this is a theoretical concern based on how these work, and it really only becomes likely with big, repeated doses of baking soda (think tablespoons daily). An occasional antacid pinch is unlikely to matter, but since amikacin is serious medicine, let your pharmacist or doctor know what you're taking.

What Is Amikacin Oral Inhalation Used For?

Brand names: Amikin, Arikayce

Arikayce is approved for adults with refractory Mycobacterium avium complex (MAC) lung disease — a serious bacterial lung infection that has not responded to at least 6 consecutive months of a standard multidrug antibiotic regimen. It is used as part of a combination treatment plan, not on its own.

This approval is limited: Arikayce is intended only for adults who have few or no other treatment options. It is not recommended for patients whose MAC lung disease has not yet failed standard therapy.
