Why this applies to Methyclothiazide
Methyclothiazide falls under THIAZIDE DIURETICS — the group Sodium Bicarbonate interacts with.
Thiazide diuretics increase urinary potassium excretion. Orally, use of excessive sodium bicarbonate (such as the intake of "tablespoons" of sodium bicarbonate daily or up to one box of baking soda weekly) has been associated with cases of hypokalemia. Furthermore, the most common complication of intravenous sodium bicarbonate is hypokalemia.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: methyclothiazide is a water pill that already nudges your body to flush out potassium in your urine. Baking soda (sodium bicarbonate), if you take a lot of it, can lower potassium too. Stack the two together and your potassium could dip lower than it should, which we call hypokalemia. Low potassium can show up as muscle cramps or weakness, fatigue, or an irregular heartbeat.
The good news: this is a theoretical concern based on how the two work, and it mostly applies to large amounts of baking soda (think tablespoons daily or most of a box in a week). An occasional pinch to settle heartburn is unlikely to cause trouble. Still, baking soda is loaded with sodium, which can fight against your blood pressure goal. Methyclothiazide is used to treat high blood pressure. Methyclothiazide is also used to treat edema (fluid retention; excess fluid held in body tissues) caused by various medical problems, including heart, kidney, and liver disease and to treat edema caused by using certain medications including estrogen and corticosteroids. Methyclothiazide is in a class of medications called diuretics ('water pills'). It works by causing the kidneys to get rid of unneeded water and salt from the body into the urine.
