CORTICOSTEROIDS
Why this applies to Clobetasol
Clobetasol falls under CORTICOSTEROIDS — the group Sodium Bicarbonate interacts with.
Orally, use of excessive sodium bicarbonate (such as the intake of "tablespoons" of sodium bicarbonate daily or up to one box of baking soda weekly) has been associated with cases of hypokalemia. Furthermore, the most common intravenous complication of sodium bicarbonate is hypokalemia. Some glucocorticoids (corticosteroids) can also cause hypokalemia by causing sodium retention, resulting in compensatory renal potassium excretion. It is most common with hydrocortisone, cortisone, and fludrocortisone, followed by prednisone and prednisolone.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the good news for you: this interaction is really about steroids you swallow, and clobetasol isn't one of those. It's a topical steroid you rub onto your skin or scalp for things like eczema and psoriasis. Very little of it gets into your bloodstream when used as directed, so the worry about low potassium (hypokalemia) from combining steroids with large amounts of baking soda doesn't really translate to a cream or foam.
The concern on file is theoretical and tied to taking big oral doses of sodium bicarbonate (think tablespoons daily) along with pills like prednisone. For normal topical clobetasol use, an occasional antacid dose of baking soda isn't something I'd lose sleep over. If you're using clobetasol over large areas for a long time, or you have heart or kidney issues, just run it by your pharmacist.
