Sodium Phenylbutyrate falls under SODIUM-CONTAINING DRUGS — the group Sodium Bicarbonate interacts with.

Concomitant use of sodium-containing drugs with additional sodium from dietary or supplemental sources may increase the risk of hypernatremia and long-term sodium-related adverse effects.
The Chronic Disease Risk Reduction (CDRR) intake level of 2.3 grams of sodium daily indicates the intake at which it is believed that chronic disease risk increases for the apparently healthy population. Some medications contain high quantities of sodium. When used in conjunction with sodium bicarbonate, the CDRR may be exceeded. Additionally, concomitant use may increase the risk for hypernatremia; this risk is highest in the elderly and people with other risk factors for electrolyte disturbances.

Here's the simple way to think about this one. Sodium phenylbutyrate already carries a hefty amount of sodium built right into it, and it's often dosed several times a day for urea cycle disorders. Sodium bicarbonate (baking soda) is also loaded with sodium. So the worry isn't about one canceling the other out, it's that you're stacking two sources of sodium on top of each other.

Adding them together could push your total sodium too high, which may lead to swelling, rising blood pressure, extra thirst, or fluid buildup. This concern is mostly theoretical and based on the chemistry, not strong studies, so don't panic. But it matters more if you have heart, kidney, or blood pressure problems, or if you're older. Please check with your pharmacist or doctor before using baking soda regularly alongside your medication.
