Why this applies to Oxtriphylline

Oxtriphylline falls under METHYLXANTHINES — the group Sodium Bicarbonate interacts with.

Theoretically, sodium bicarbonate may increase the risk of hypokalemia in patients taking methylxanthines.
Orally, use of excessive sodium bicarbonate (such as the intake of "tablespoons" of sodium bicarbonate daily or up to one box of baking soda weekly) has been associated with cases of hypokalemia. Furthermore, the most common complication of intravenous sodium bicarbonate is hypokalemia. Theophylline and related drugs can reduce serum potassium levels, possibly by increasing intracellular uptake of potassium. Hypokalemia is most likely to occur after acute overdose of these drugs. However, reduced potassium levels can occur with therapeutic doses, and the incidence and degree of hypokalemia increases with increasing serum theophylline levels.

Here's the gist for Oxtriphylline (a methylxanthine, related to theophylline): both this medicine and large amounts of sodium bicarbonate can independently nudge your potassium level lower. Oxtriphylline tends to push potassium into your cells, and overdoing baking soda has been linked to low potassium too. Stack them and, in theory, the dip could be a little bigger. Low potassium can show up as muscle weakness, cramps, tiredness, or an irregular heartbeat.

I want to be honest that this concern is theoretical, based on how these substances work rather than firm patient studies. An occasional antacid pinch of baking soda is unlikely to matter. The worry is heavy, regular use (tablespoons daily). Please check with your pharmacist or doctor before using baking soda routinely, and don't change your Oxtriphylline on your own.
