Methylprednisolone falls under CORTICOSTEROIDS — the group Sodium Bicarbonate interacts with.

Theoretically, sodium bicarbonate may increase the risk of hypokalemia in patients taking corticosteroids.
Orally, use of excessive sodium bicarbonate (such as the intake of "tablespoons" of sodium bicarbonate daily or up to one box of baking soda weekly) has been associated with cases of hypokalemia. Furthermore, the most common intravenous complication of sodium bicarbonate is hypokalemia. Some glucocorticoids (corticosteroids) can also cause hypokalemia by causing sodium retention, resulting in compensatory renal potassium excretion. It is most common with hydrocortisone, cortisone, and fludrocortisone, followed by prednisone and prednisolone.

Here's the gist: both methylprednisolone and large amounts of baking soda can nudge your potassium levels downward. Steroids like Medrol make your body hold onto sodium, and your kidneys trade that off by dumping potassium. Heavy sodium bicarbonate use can lower potassium too, so stacking the two could add up.

Honestly, though, this is a theoretical concern based on how these work, not strong proof. The bigger worry is only if you're taking baking soda in large amounts (think tablespoons daily or close to a box a week), not an occasional pinch for heartburn. If potassium drops, you might feel muscle weakness, cramps, or a fluttery heartbeat. If you use sodium bicarbonate regularly while on Medrol, mention it to your pharmacist or doctor so they can keep an eye on things.

What Is Methylprednisolone Used For?

Brand names: Medrol, Medrol Dosepak

Methylprednisolone tablets (Medrol, Methylpred DP) are used to treat a broad set of conditions including endocrine disorders, rheumatic and joint conditions, autoimmune diseases such as systemic lupus erythematosus, severe allergic states, skin diseases, eye inflammation, breathing disorders like asthma, blood and kidney diseases, and certain cancers.

Methylprednisolone acetate injection (Depo-Medrol and others) is used by intramuscular injection when oral therapy isn't feasible, and can also be given directly into joints, soft tissue, or lesions to treat local inflammation from conditions such as rheumatoid arthritis, bursitis, and skin disorders. The injection is not approved for intrathecal (spinal) use.
