CHLORPROPAMIDE (Diabinese)

Theoretically, sodium bicarbonate might reduce the levels and clinical effects of chlorpropamide.
The elimination of chlorpropamide by the kidneys depends strongly on urine pH. At a pH of 5, the renal clearance of chlorpropamide ranges from 0.5 to 3 mL/hr. At a pH of 8, renal clearance of chlorpropamide ranges from 500 to 1000 mL/hr. When taken in combination with oral sodium bicarbonate, the elimination half-life of chlorpropamide is shortened from 49.7 to 12.8 hours and urinary excretion of chlorpropamide is increased four-fold.

Here's the gist with chlorpropamide and baking soda. Your kidneys clear chlorpropamide out faster when your urine is more alkaline (less acidic), and sodium bicarbonate is a base that does exactly that. In studies, taking sodium bicarbonate cut chlorpropamide's half-life dramatically (from about 50 hours down to around 13 hours), meaning the drug leaves your body much quicker. The practical concern is that your chlorpropamide may not work as well, so your blood sugar could creep up higher than usual.

This isn't a prodrug, so faster removal simply means less effect, not more. An occasional antacid dose for heartburn likely won't matter much, but regular or high-dose sodium bicarbonate could. If you take baking soda routinely, please check with your pharmacist or doctor and keep an eye on your blood sugar readings.
