Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: coffee charcoal works like a sponge in your gut. That porous, charred surface can grab onto medicine sitting in your stomach and intestines before your body has a chance to absorb it. Oxtriphylline (Choledyl) is a breathing medicine that relaxes your airways, so if the charcoal soaks some of it up, you may get less into your bloodstream than you need.

With a drug like this, getting too little can mean your asthma or COPD isn't as well controlled, with more wheezing or shortness of breath. The easy fix most people use is timing: keep the two doses at least two hours apart. This is based on how charcoal behaves rather than firm studies, so check with your pharmacist or doctor before combining them.

From our pharmacist’s overview on the Oxtriphylline page

“Ephedra tops the worry list for anyone taking oxtriphylline (Choledyl), a theophylline-based bronchodilator that opens airways in asthma and chronic lung disease. Ephedra and the related herb Sida cordifolia stack stimulant effects onto a methylxanthine drug like this one, raising the risk of serious reactions, and activated charcoal,…”

About Coffee Charcoal

What is Coffee Charcoal?

  • Diarrhea
  • Inflammatory bowel conditions
  • Irritable bowel syndrome (IBS)
  • Mild digestive upset
  • Wound and skin care (topical, traditional)

Coffee charcoal is made by roasting green coffee beans until they turn black, creating a charcoal-like material used traditionally for diarrhea and digestive complaints. Scientific evidence supporting its benefits is very limited, and it should not replace proven treatments. Talk with your pharmacist or doctor before using it, especially if you have a chronic gut condition.
