Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing about tobramycin that actually works in your favor: the way you're most likely getting it (an injection, or an inhaled mist for lung infections like in cystic fibrosis) doesn't really mix with anything in your stomach. Coffee charcoal works by binding to things in the gut, so it can only grab a drug that's swallowed and absorbed through the intestines. Tobramycin is not taken as an oral pill for that reason, so charcoal has little to nothing to latch onto.

This concern is really a general, theoretical one for swallowed medicines, not something proven for tobramycin specifically. Still, if you're ever given an oral medication, spacing charcoal a couple hours apart is smart. Check with your pharmacist about your exact form to be safe.

From our pharmacist’s overview on the Tobramycin page

“Kidney protection is the first thing to understand with tobramycin (Nebcin, Tobi Podhaler, Bethkis, Kitabis PAK), an aminoglycoside antibiotic that can stress the kidneys even on its own, so this list warrants genuine care rather than a quick glance. The worst pairings, Siberian cocklebur and asarabacca, can injure the kidneys themselv…”

About Coffee Charcoal

What is Coffee Charcoal?

  • Diarrhea
  • Inflammatory bowel conditions
  • Irritable bowel syndrome (IBS)
  • Mild digestive upset
  • Wound and skin care (topical, traditional)

Coffee charcoal is made by roasting green coffee beans until they turn black, creating a charcoal-like material used traditionally for diarrhea and digestive complaints. Scientific evidence supporting its benefits is very limited, and it should not replace proven treatments. Talk with your pharmacist or doctor before using it, especially if you have a chronic gut condition.
