Bifidobacterium Longum and Vancomycin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIBIOTIC DRUGS

Why this applies to Vancomycin

Vancomycin falls under ANTIBIOTIC DRUGS — the group Bifidobacterium Longum interacts with.

Theoretically, taking Bifidobacterium longum with antibiotic drugs might decrease the effectiveness of B. longum.
Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind: this interaction works against the probiotic, not against your vancomycin. Bifidobacterium longum is a live, friendly bacterium, and vancomycin is an antibiotic whose whole job is to kill bacteria in the gut. So if you swallow them at the same time, the vancomycin can wipe out a good chunk of the probiotic organisms before they have a chance to do anything useful.

Your vancomycin will still work fine for the colitis it was prescribed for, so don't worry on that end. This is more of a 'don't waste your probiotic' situation. A simple fix is to space them out by at least 2 hours. Since this is based on how the two are expected to behave rather than hard studies, it's worth a quick chat with your pharmacist to time things right.

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Vancomycin page

“Vancomycin (Vancocin) treats serious bacterial infections, and its oral form is a mainstay for C. diff in the gut, so read this list as a genuine safety check, not background noise.”

About Bifidobacterium Longum

What is Bifidobacterium Longum?

  • Digestive health and regularity
  • Irritable bowel syndrome (IBS)
  • Diarrhea (including antibiotic-related)
  • Immune support
  • Mood and stress (gut-brain axis)

Bifidobacterium longum is a 'friendly' bacterium found naturally in the human gut and used as a probiotic. About Vancomycin

What Is Vancomycin Used For?

Brand names: Vancocin, Vancoled

Vancomycin is used to fight serious bacterial infections caused by certain hard-to-treat bacteria, including methicillin-resistant Staphylococcus aureus (MRSA). When given intravenously (by IV), it treats conditions like septicemia (a serious bloodstream infection), infective endocarditis (infection of the heart valves), skin and skin structure infections, bone infections, and lower respiratory tract infections.

When taken by mouth — as capsules or oral solution (brands include Vancocin and Firvanq) — vancomycin treats Clostridioides difficile-associated diarrhea (C. diff) and staphylococcal enterocolitis (a gut infection caused by Staphylococcus aureus). It's important to know that the oral form does not work for bloodstream or other serious infections, and the IV form does not work for C. diff or gut infections — each route has its own specific uses.
