The Lactobacillus acidophilus monograph (which covers L. plantarum 299v) shows the ingredient is possibly effective for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis. For atopic disease (allergic conditions), it's rated possibly ineffective.
For acute respiratory distress syndrome, we don't have enough reliable evidence to rate it. We hold no effectiveness data for Lactobacillus rhamnosus Rosell-11ND.
How safe is it?
Well-documented data
Lactobacillus plantarum 299v is generally well tolerated in healthy adults and children. Some people taking it alongside other probiotics have reported mild gastrointestinal discomfort — bloating, gas, abdominal pain, or diarrhea — though these events are uncommon.
In rare cases, the live bacteria have been associated with serious infections in people with compromised immune systems; most of those cases involved bacteria that had moved from other parts of the body. Since data are limited, check with your doctor before starting.
For pregnancy, the available evidence suggests L. plantarum 299v is possibly safe, though you should still confirm with your doctor before use. We have no data on breastfeeding safety for this ingredient, nor any data on either concern for L. rhamnosus Rosell-11ND.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
